CDA Profile Update Request

Parents/Guardians can now verify and update their profile and their child’s profile by visiting My CDA Profile and logging in with their credentials.
If you need to be added, removed, or any of the non-editable information on file needs to be updated, please fill out and submit the form below.

Allow about one week for the changes to reflect or to receive a response.

* Please remember that updating your information on cdavenice.org will NOT update your child’s emergency cards within the Administration Office. Please assure that you always have an up-to-date emergency card on file at the office.
